David is a VA and United Healthcare provider, and also accepts workers comp insurance. Many other companies like Anthem, Aetna, Blue Shield, and Cigna provide out-of-network benefits once you meet your deductible.
To find out if you have acupuncture benefits as part of your plan, simply email David with:
-Your name as it appears on your insurance card (If you are not the Primary on the account, please include the Primary’s name and DOB as well)
-Your date of birth
-Your insurance company name and ID #

IF YOU HAVE WORKERS COMP INSURANCE, simply have your Primary Doctor create a referral for David’s office, Evolution Wellness.
IF YOU ARE A VETERAN, contact your Primary Care Physician and ask them for a referral to see David for acupuncture. They can fax the referral to the Community Care Network (CCN) at 303-398-5550, ATTN POD 1.
